Healthgram Care Management System (CMS) is an electronic medical record and practice management application designed to support on-site medical clinics and remote care coordination. CMS integrates claim data, Rx data, and Lab data and patient encounter information into one integrated system. Patient encounter information includes on-site medical delivery, disease management, case management, prevention management and care coordination.
