We arrange for placement of a healthcare professional on a company's premises—from nurse practitioners who provide preventative and acute care to disease management nurses who monitor the progress of treatments. Clinic - Setup and Monitoring
Experience and Efficiency. Primary PhysicianCare provides all the necessary services and infrastructure to set up and to operate an On-site clinic in the workplace. Our proprietary web-based management software ensures efficient, secure, and paperless transactions Trust us to:
- Recruit, interview and hire contract employees to staff the clinic
- Provide all necessary legal and liability documentation
- Configure office space and install computer equipment
- Furnish all medical supplies
- Deliver Cholestech and other diagnostic medical equipment
- Coordinate with laboratory for specimen pick up and processing
- Provide access to our Care Management System™—Primary PhysicianCare's practice management software
- Supply health risk assessment forms for screenings
- Provide condition-specific informational brochures
- Assist with member communications on clinic procedures and availability
- Report on clinic activity and costs

Roles of Medical Professionals
Fully-integrated Management . Below is a sample of the fully-integrated services offered by our medical professionals:
- Refer patients to their primary care physicians when appropriate
- Assist in locating an in-network primary care physician when needed
- Deliver patient education and disease management interventions
- Coordinate and perform health and wellness screenings
- Identify individuals who might benefit from disease management
- Maintain a stock of approved over-the-counter medications and basic first aid supplies, administering care to employees within the standards permitted for licensed nurses
- Monitor and evaluate intervention effectiveness
- Refer members to catastrophic case management as appropriate
- Assure that On-site management form is completed (either on paper or via Healthgram) on each patient
