Care Coordination

Healthy Mothers and InfantsMaternity Management identifies the risk levels of mothers-to-be and their pregnancies.  This information enables us to determine an appropriate level of care and to develop a customized care plan. Our experienced obstetrical nurses offer insightful guidance through one-on-one support and care coordination.

Maternity Management Highlights :

  • Education, counseling and health promotion information
  • Patient education on the abnormal symptoms of pregnancy and information on behavior modification
  • A case manager for the patient to contact if abnormal symptoms appear
  • An alert to case managers regarding pregnant patients with abnormal symptoms or risk factors
  • A nurse case manager experienced in prenatal care to answer questions up to six weeks after delivery 
  • Data management and reporting of program activity including outcomes reports

 

Why Maternity Management?

Importance of Quality Care.  Unfortunately, more than 470,000 infants (about 1 in 8 babies) are born prematurely in the United States each year. Premature births place a severe emotional strain on American families and a severe financial strain on our healthcare system.  Maternity Management is a proactive resource to reduce the stress associated with premature births on both families and employers.
