On-site Management
The key to maximizing the effectiveness of On-site healthcare providers is to establish chronic disease management and patient education as their top priorities. These functions have the best long-term impact on employee heath and control of health plan expenses. Primary PhysicianCare has many years of experience placing healthcare professionals On-site and clearly defining their responsibilities in a way that can be tracked, documented and reported. This is accomplished in part because our practitioners use our proprietary software to perform patient interventions.
Recommended duties for On-site practitioners include:
- Refer patients to their primary care physicians when appropriate.
- Assist patients in locating an in-network primary care physician when needed.
- Participate in the delivery of patient education and disease management interventions.
- Coordinate health and wellness screenings.
- Perform wellness screenings by obtaining basic physical status information concerning employees’ vital signs, weight and laboratory screenings; and identify individuals who are potential disease management patients. Review screening results (compared to standard assessment values), notify employees of abnormal results, and provide appropriate information for employees who need to consult with a physician or to seek other healthcare services.
- Monitor situations requiring physician follow-up and encourage employees to obtain necessary consultations and treatments.
- Document patients’ conditions including disease protocols, understanding of the condition and awareness of self-care techniques.
- Maintain approved over-the-counter medications and basic first aid supplies; and administer care to employees within the standards permitted for licensed nurses.
- Maintain confidentiality of medical and other records generated from this program.
- Counsel employees concerning compliance with disease management programs.
- Meet with individual employees upon their request concerning their DM program.
- Use motivational interviews to facilitate health behavior changes.
- Monitor and evaluate effectiveness of interventions and/or health coaching plans and modify as needed.
- Refer members to catastrophic case management as appropriate.
- Assure that On-site management form is completed on each patient.
